MY Solutions

Our team consists of board-certified nurse practitioners and nurse case managers who offer our seniors primary care, endocrine services, as well as psychiatric care, all at the comfort of your home. Whether you live in an assisted living facility or a private residence, we offer door-to-door service. We provide personalized expert care to every patient that we have the privilege of serving.

 

Services: Telehealth, In-Home Care, Assisted Facility Care, Primary Care, Endocrinology, Gerontology, Psychiatry, Nursing Services, and Case Management.

MY Solutions offers alternative services to patients who are homebound, whether temporary or permanent.  For those in need of temporary care and unable to make it to their provider due to physical or cognitive conditions, in-home interim care is also offered. With our awesome community partners, our goal is to keep our patients safe and happy at home. All by ensuring our patients’ needs are met through partnering with only the best community resources, by building relationships, and forging trust. Our mission is to improve the health and well-being of our patients and the greater community.

Office Hours

MY Solutions is open 9 - 5 from Monday to Thursday and closed on Fridays and the Weekend, offering on-call services.  We pride ourselves as a practice to reassure patients and families. We take all major health insurances, proudly serving Hobe Sound through the Sebastian area.
